powerpc/perf: Dereference BHRB entries safely
commit f41d84dddc66b164ac16acf3f584c276146f1c48 upstream.
It's theoretically possible that branch instructions recorded in
BHRB (Branch History Rolling Buffer) entries have already been
unmapped before they are processed by the kernel. Hence, trying to
dereference such memory location will result in a crash. eg:

powerpc/perf/hv-24x7: Fix incorrect comparison in memord
[ Upstream commit 05c14c03138532a3cb2aa29c2960445c8753343b ] In the hv-24x7 code there is a function memord() which tries to implement a sort function return -1, 0, 1. However one of the conditions is incorrect, such that it can never be true, because we will have already returned. I don't believe there is a bug in practice though, because the comparisons are an optimisation prior to calling memcmp(). Fix it by swapping the second comparision, so it can be true. powerpc/opal: Fix EBUSY bug in acquiring tokens

The current code checks the completion map to look for the first token
that is complete. In some cases, a completion can come in but the
token can still be on lease to the caller processing the completion.
If this completed but unreleased token is the first token found in the
bitmap by another tasks trying to acquire a token, then the
__test_and_set_bit call will fail since the token will still be on
lease. The acquisition will then fail with an EBUSY.
This patch reorganizes the acquisition code to look at the
opal_async_token_map for an unleased token. If the token has no lease
it must have no outstanding completions so we should never see an
EBUSY, unless we have leased out too many tokens. Since
opal_async_get_token_inrerruptible is protected by a semaphore, we
will practically never see EBUSY anymore.

powerpc/powernv/cpufreq: Fix the frequency read by /proc/cpuinfo

The call to /proc/cpuinfo in turn calls cpufreq_quick_get() which
returns the last frequency requested by the kernel, but may not
reflect the actual frequency the processor is running at. This patch
makes a call to cpufreq_get() instead which returns the current
frequency reported by the hardware.

powerpc/64: Fix checksum folding in csum_tcpudp_nofold and ip_fast_csum_nofold
commit b492f7e4e07a28e706db26cf4943bb0911435426 upstream. These functions compute an IP checksum by computing a 64-bit sum and folding it to 32 bits (the "nofold" in their names refers to folding down to 16 bits). However, doing (u32) (s + (s >> 32)) is not sufficient to fold a 64-bit sum to 32 bits correctly. The addition can produce a carry out from bit 31, which needs to be added in to the sum to produce the correct result. To fix this, we copy the from64to32() function from lib/checksum.c and use that. powerpc/powernv/ioda2: Gracefully fail if too many TCE levels requested
[ Upstream commit 7aafac11e308d37ed3c509829bb43d80c1811ac3 ] The IODA2 specification says that a 64 DMA address cannot use top 4 bits (3 are reserved and one is a "TVE select"); bottom page_shift bits cannot be used for multilevel table addressing either. The existing IODA2 table allocation code aligns the minimum TCE table size to PAGE_SIZE so in the case of 64K system pages and 4K IOMMU pages, we have 64-4-12=48 bits. Since 64K page stores 8192 TCEs, i.e. needs 13 bits, the maximum number of levels is 48/13 = 3 so we physically cannot address more and EEH happens on DMA accesses. This adds a check that too many levels were requested. It is still possible to have 5 levels in the case of 4K system page size. axonram: Fix gendisk handling

powerpc: Fix compiling a BE kernel with a powerpc64le toolchain
[ Upstream commit 4dc831aa88132f835cefe876aa0206977c4d7710 ] GCC can compile with either endian, but the default ABI version is set based on the default endianness of the toolchain. Alan Modra says: you need both -mbig and -mabi=elfv1 to make a powerpc64le gcc generate powerpc64 code The opposite is true for powerpc64 when generating -mlittle it requires -mabi=elfv2 to generate v2 ABI, which we were already doing. This change adds ABI annotations together with endianness for all cases, LE and BE. This fixes the case of building a BE kernel with a toolchain that is LE by default. powerpc/64: Invalidate process table caching after setting process table

When we kexec from one kernel to another, the second kernel sets its process table pointer but doesn't currently do anything to make the CPU invalidate any cached entries from the old process table. This adds a tlbie (TLB invalidate entry) instruction with parameters to invalidate caching of the process table after the new process table is installed. powerpc/64s: Initialize ISAv3 MMU registers before setting partition table
commit 371b80447ff33ddac392c189cf884a5a3e18faeb upstream.
kexec can leave MMU registers set when booting into a new kernel,
the PIDR (Process Identification Register) in particular. The boot
sequence does not zero PIDR, so it only gets set when CPUs first
switch to a userspace processes (until then it's running a kernel
thread with effective PID = 0).
This leaves a window where a process table entry and page tables are
set up due to user processes running on other CPUs, that happen to
match with a stale PID. The CPU with that PID may cause speculative
accesses that address quadrant 0 (aka userspace addresses), which will
result in cached translations and PWC (Page Walk Cache) for that
process, on a CPU which is not in the mm_cpumask and so they will not
be invalidated properly.
The most common result is the kernel hanging in infinite page fault
loops soon after kexec (usually in schedule_tail, which is usually the
first non-speculative quadrant 0 access to a new PID) due to a stale
PWC. However being a stale translation error, it could result in
anything up to security and data corruption problems.
Fix this by zeroing out PIDR at boot and kexec.

powerpc/signal: Properly handle return value from uprobe_deny_signal()

When a uprobe is installed on an instruction that we currently do not
emulate, we copy the instruction into a xol buffer and single step
that instruction. If that instruction generates a fault, we abort the
single stepping before invoking the signal handler. Once the signal
handler is done, the uprobe trap is hit again since the instruction is
retried and the process repeats.
We use uprobe_deny_signal() to detect if the xol instruction triggered
a signal. If so, we clear TIF_SIGPENDING and set TIF_UPROBE so that the
signal is not handled until after the single stepping is aborted. In
this case, uprobe_deny_signal() returns true and get_signal() ends up
returning 0. However, in do_signal(), we are not looking at the return
value, but depending on ksig.sig for further action, all with an
uninitialized ksig that is not touched in this scenario. Fix the same
by initializing ksig.sig to 0.

security/keys: add CONFIG_KEYS_COMPAT to Kconfig
commit 47b2c3fff4932e6fc17ce13d51a43c6969714e20 upstream. CONFIG_KEYS_COMPAT is defined in arch-specific Kconfigs and is missing for several 64-bit architectures : mips, parisc, tile. At the moment and for those architectures, calling in 32-bit userspace the keyctl syscall would return an ENOSYS error. This patch moves the CONFIG_KEYS_COMPAT option to security/keys/Kconfig, to make sure the compatibility wrapper is registered by default for any 64-bit architecture as long as it is configured with CONFIG_COMPAT. sched/cputime, powerpc32: Fix stale scaled stime on context switch
[ Upstream commit 90d08ba2b9b4be4aeca6a5b5a4b09fbcde30194d ] On context switch with powerpc32, the cputime is accumulated in the thread_info struct. So the switching-in task must move forward its start time snapshot to the current time in order to later compute the delta spent in system mode. This is what we do for the normal cputime by initializing the starttime field to the value of the previous task's starttime which got freshly updated. But we are missing the update of the scaled cputime start time. As a result we may be accounting too much scaled cputime later. Fix this by initializing the scaled cputime the same way we do for normal cputime. powerpc/64: Don't try to use radix MMU under a hypervisor

Currently, if the kernel is running on a POWER9 processor under a
hypervisor, it will try to use the radix MMU even though it doesn't have
the necessary code to use radix under a hypervisor (it doesn't negotiate
use of radix, and it doesn't do the H_REGISTER_PROC_TBL hcall). The
result is that the guest kernel will crash when it tries to turn on the
MMU.
This fixes it by looking for the /chosen/ibm,architecture-vec-5
property, and if it exists, clears the radix MMU feature bit, before we
decide whether to initialize for radix or HPT. This property is created
by the hypervisor as a result of the guest calling the
ibm,client-architecture-support method to indicate its capabilities, so
it will indicate whether the hypervisor agreed to us using radix.
Systems without a hypervisor may have this property also (for example,
skiboot creates it), so we check the HV bit in the MSR to see whether we
are running as a guest or not. If we are in hypervisor mode, then we can
do whatever we like including using the radix MMU.
The reason for using this property is that in future, when we have
support for using radix under a hypervisor, we will need to check this
property to see whether the hypervisor agreed to us using radix.

This happens because when using the global KVM fd with
KVM_CHECK_EXTENSION, kvm_vm_ioctl_check_extension() gets
called with a NULL kvm argument, which gets dereferenced
in is_kvmppc_hv_enabled(). Spotted while reading the code.
Let's use the hv_enabled fallback variable, like everywhere
else in this function.

powerpc/tm: Fix illegal TM state in signal handler

Currently it's possible that on returning from the signal handler
through the restore_tm_sigcontexts() code path (e.g. from a signal
caught due to a `trap` instruction executed in the middle of an HTM
block, or a deliberately constructed sigframe) an illegal TM state
(like TS=10 TM=0, i.e. "T0") is set in SRR1 and when `rfid` sets
implicitly the MSR register from SRR1 register on return to userspace
it causes a TM Bad Thing exception.
That illegal state can be set (a) by a malicious user that disables
the TM bit by tweaking the bits in uc_mcontext before returning from
the signal handler or (b) by a sufficient number of context switches
occurring such that the load_tm counter overflows and TM is disabled
whilst in the signal handler.
This commit fixes the illegal TM state by ensuring that TM bit is
always enabled before we return from restore_tm_sigcontexts(). A small
comment correction is made as well.

powerpc/64s: Use emergency stack for kernel TM Bad Thing program checks

When using transactional memory (TM), the CPU can be in one of six
states as far as TM is concerned, encoded in the Machine State
Register (MSR). Certain state transitions are illegal and if attempted
trigger a "TM Bad Thing" type program check exception.
If we ever hit one of these exceptions it's treated as a bug, ie. we
oops, and kill the process and/or panic, depending on configuration.
One case where we can trigger a TM Bad Thing, is when returning to
userspace after a system call or interrupt, using RFID. When this
happens the CPU first restores the user register state, in particular
r1 (the stack pointer) and then attempts to update the MSR. However
the MSR update is not allowed and so we take the program check with
the user register state, but the kernel MSR.
This tricks the exception entry code into thinking we have a bad
kernel stack pointer, because the MSR says we're coming from the
kernel, but r1 is pointing to userspace.
To avoid this we instead always switch to the emergency stack if we
take a TM Bad Thing from the kernel. That way none of the user
register values are used, other than for printing in the oops message.
This is the fix for CVE-2017-1000255.

powerpc/ftrace: Pass the correct stack pointer for DYNAMIC_FTRACE_WITH_REGS

For DYNAMIC_FTRACE_WITH_REGS, we should be passing-in the original set
of registers in pt_regs, to capture the state _before_ ftrace_caller.

in pt_regs. Also, use SAVE_10GPRS() to simplify the code.

powerpc/tm: Flush TM only if CPU has TM feature

Commit cd63f3c ("powerpc/tm: Fix saving of TM SPRs in core dump")
added code to access TM SPRs in flush_tmregs_to_thread(). However
flush_tmregs_to_thread() does not check if TM feature is available on
CPU before trying to access TM SPRs in order to copy live state to
thread structures. flush_tmregs_to_thread() is indeed guarded by
CONFIG_PPC_TRANSACTIONAL_MEM but it might be the case that kernel
was compiled with CONFIG_PPC_TRANSACTIONAL_MEM enabled and ran on
a CPU without TM feature available, thus rendering the execution
of TM instructions that are treated by the CPU as illegal instructions.
The fix is just to add proper checking in flush_tmregs_to_thread()
if CPU has the TM feature before accessing any TM-specific resource,
returning immediately if TM is no available on the CPU. Adding
that checking in flush_tmregs_to_thread() instead of in places
where it is called, like in vsr_get() and vsr_set(), is better because
avoids the same problem cropping up elsewhere.

powerpc/pseries: Fix parent_dn reference leak in add_dt_node()

node to be added. If the call to dlpar_configure_connector() fails

KVM: PPC: Book3S HV: Protect updates to spapr_tce_tables list

Al Viro pointed out that while one thread of a process is executing
in kvm_vm_ioctl_create_spapr_tce(), another thread could guess the
file descriptor returned by anon_inode_getfd() and close() it before
the first thread has added it to the kvm->arch.spapr_tce_tables list.
That highlights a more general problem: there is no mutual exclusion
between writers to the spapr_tce_tables list, leading to the
possibility of the list becoming corrupted, which could cause a
host kernel crash.
To fix the mutual exclusion problem, we add a mutex_lock/unlock
pair around the list_del_rce in kvm_spapr_tce_release().
If another thread does guess the file descriptor returned by the
anon_inode_getfd() call in kvm_vm_ioctl_create_spapr_tce() and closes
it, its call to kvm_spapr_tce_release() will not do any harm because
it will have to wait until the first thread has released kvm->lock.
The other things that the second thread could do with the guessed
file descriptor are to mmap it or to pass it as a parameter to a
KVM_DEV_VFIO_GROUP_SET_SPAPR_TCE ioctl on a KVM device fd. An mmap
call won't cause any harm because kvm_spapr_tce_mmap() and
kvm_spapr_tce_fault() don't access the spapr_tce_tables list or
the kvmppc_spapr_tce_table.list field, and the fields that they do use
have been properly initialized by the time of the anon_inode_getfd()
call.
The KVM_DEV_VFIO_GROUP_SET_SPAPR_TCE ioctl calls
kvm_spapr_tce_attach_iommu_group(), which scans the spapr_tce_tables
list looking for the kvmppc_spapr_tce_table struct corresponding to
the fd given as the parameter. Either it will find the new entry
or it won't; if it doesn't, it just returns an error, and if it
does, it will function normally. So, in each case there is no
harmful effect.

Nixiaoming pointed out that there is a memory leak in kvm_vm_ioctl_create_spapr_tce() if the call to anon_inode_getfd() fails; the memory allocated for the kvmppc_spapr_tce_table struct is not freed, and nor are the pages allocated for the iommu tables. In addition, we have already incremented the process's count of locked memory pages, and this doesn't get restored on error. David Hildenbrand pointed out that there is a race in that the function checks early on that there is not already an entry in the stt->iommu_tables list with the same LIOBN, but an entry with the same LIOBN could get added between then and when the new entry is added to the list. This fixes all three problems. To simplify things, we now call anon_inode_getfd() before placing the new entry in the list. The check for an existing entry is done while holding the kvm->lock mutex, immediately before adding the new entry to the list. Finally, on failure we now call kvmppc_account_memlimit to decrement the process's count of locked memory pages. powerpc: Fix DAR reporting when alignment handler faults
commit f9effe925039cf54489b5c04e0d40073bb3a123d upstream. Anton noticed that if we fault part way through emulating an unaligned instruction, we don't update the DAR to reflect that. The DAR value is eventually reported back to userspace as the address in the SEGV signal, and if userspace is using that value to demand fault then it can be confused by us not setting the value correctly. This patch is ugly as hell, but is intended to be the minimal fix and back ports easily. There is no guarantee that the various isync's involved with the context switch will order the update of the CPU mask with the first TLB entry for the new context being loaded by the HW. Be safe here and add a memory barrier to order any subsequent load/store which may bring entries into the TLB. The corresponding barrier on the other side already exists as pte updates use pte_xchg() which uses __cmpxchg_u64 which has a sync after the atomic operation. powerpc: Fix VSX enabling/flushing to also test MSR_FP and MSR_VEC

VSX uses a combination of the old vector registers, the old FP
registers and new "second halves" of the FP registers.
Thus when we need to see the VSX state in the thread struct
(flush_vsx_to_thread()) or when we'll use the VSX in the kernel
(enable_kernel_vsx()) we need to ensure they are all flushed into
the thread struct if either of them is individually enabled.
Unfortunately we only tested if the whole VSX was enabled, not if they
were individually enabled.

powerpc/64: Fix __check_irq_replay missing decrementer interrupt
commit 3db40c312c2c1eb2187c5731102fa8ff380e6e40 upstream.
If the decrementer wraps again and de-asserts the decrementer
exception while hard-disabled, __check_irq_replay() has a test to
notice the wrap when interrupts are re-enabled.
The decrementer check must be done when clearing the PACA_IRQ_HARD_DIS
flag, not when the PACA_IRQ_DEC flag is tested. Previously this worked
because the decrementer interrupt was always the first one checked
after clearing the hard disable flag, but HMI check was moved ahead of
that, which introduced this bug.
This can cause a missed decrementer interrupt if we soft-disable
interrupts then take an HMI which is recorded in irq_happened, then
hard-disable interrupts for > 4s to wrap the decrementer.

powerpc/tm: Fix saving of TM SPRs in core dump

Currently flush_tmregs_to_thread() does not save the TM SPRs (TFHAR,
TFIAR, TEXASR) to the thread struct, unless the process is currently
inside a suspended transaction.
If the process is core dumping, and the TM SPRs have changed since the
last time the process was context switched, then we will save stale
values of the TM SPRs to the core dump.
Fix it by saving the live register state to the thread struct in that
case.
